Dear Martul, This is an important contribution to our understanding of the nature of allograft rejection by the immune system of the recipient. I have one comment. Did you specifically look for neutrophils in the infiltrate as I could not find the results of neutrophils in your presentation. We often see many neutrophils in acute cellular rejection in our cases. Even, sometimes, tubular microabscess do occur with a predominant picture of oedema and lymphocyte rich infiltrates typical of alloimmune rejection. In such cases, we do recommend urine culture to rule out infection but it is our experience that most of these turn out to be rejection processes rather than a combination of rejection and infection. How often do you come across this finding.
